To any parent that wishes I take photos of their childs event that is under 18 years of age, I need not only you but every single parent/guardian from the team to sign this form. I understand the importance of parents granting me consent to take photos of their children. I am not only ensuring I am doing my job legally but making sure that the photos I take of your children are in safe hands.

If not every parent/guardian involved with the childs event signs this form, I do not feel comfortable doing the shoot if I do not have everyones consent. Period. I am willing to lose an opportunity if that means I know the children are safe,
